Medical Release Form

Members must fill out this form to be authorized to attend events sponsored by Key Club International or Cali-Nev-Ha District. In the case of medical emergency, it is used to access your emergency and medical information to receive proper treatment.

Photo Release Form

Members must fill out this form to allow Key Club to use their picture, image, name, or any references to them for any Key Club promotion and activity.  They can withdraw this authorization at any time.

COVID Liability Form

Members must fill out this form to be fully aware of the possible COVID risks when attending in-person events. They are agreeing to voluntarily give up some legal rights, including the right to sue.
